ADMISSION POLICIES


Scope of Care:

 

 Our facility offers assistance with all of the activities of daily living such as, walking, eating, dressing, bathing, toileting, incontinence care, and transfer between bed and chair.  We accept clients that require up to a one-person assisted transfer.  We currently do not accept bed-ridden clients, clients requiring two-person transfers, or Alzheimer clients that are in the severe stage of the disease that have physically violent episodes at the time of admission.  If a resident’s care progresses to any of the levels as described above, then a care-conference is held to determine if placement in a skilled-nursing or other facility is necessary.  We do end of life care if the above criteria is met and we can keep the resident comfortable within the care limits we are able to provide as according to regulations and their plan of care.

 

Additional Services:

 

Our facility also offers medication monitoring, transportation, housekeeping and laundry service, and scheduled daily activities.  We offer private rooms, each with a toilet and sink, and three nutritious meals plus two snacks per day.  Bathrooms are handicap accessible.

 

Fees:

Billing is done at the Medicaid rate for Choice clients plus room and board. Our daily rate is $209.00.    

 

 Staffing:

Our staff is certified as either Certified Nursing Assistants or Personal Care Attendants.  A staff person is on duty 24 hours per day.  Our staff ratio for 8 people is two to three caregivers for the day and swing shifts, one for the night shift. Staffing levels may fluctuate depending on the level of care required for the current residents.
